What to do in case of claims?

NB! Before dental treatment, print out the claim submitting form (mandatory for policyholders who have joined to insurance contract terms and conditions HRK-4/2020, recommended for other policyholders).

Fill in the claim form below with the help of dental invoice or with the help of your dentist.

CLAIM SUBMITTING FORM

NB! According to clause 7.2 of the terms and conditions of the dental insurance contract HRK-4/2020, certain treatments are indemnified only if there is a claims handler’s confirmation before starting a treatment.

For a quicker process please ask your dentist to fill in the pre-authorization form below to submit your treatment plan.

Receive dental treatment

Recieve dental treatment at your preferred dental clinic.
Pay for the treatment performed. In case of card payment, keep a copy of the payment order.
Fill in the claim submitting form with the help of dental invoice or with the help of your dentist.

Enter to claims handling portal

Enter claims portal via this website menu option "Claims"
When you first log in, do register yourself as a portal user. To enter every next time please use only your personal code and password you created during the registration process.

Upload documents to portal

Upload the dental invoice, claim form (mandatory for policyholders who have joined to insurance contract terms and conditions HRK-4/2020, recommended for other policyholders), payment proof and treatment record if requested to claims portal by following the on-screen instructions.

NB! Without a tooth number identifying the tooth on which the procedure was done, the claim is invalid!

Wait for the decision

Wait for the claims administrator's decision. The processing of your claim may take up to 3 weeks.
Keep track of your claim status in the claims portal yourself.
An indemnity notice will be sent to your email address to inform you of the decision of the claims handling process.

Payment of indemnity

The insurance indemnity will be credited to your bank account within 5 working days of receipt of the notice of claim payment.
